Note:

Web browser in PS4 doesn't support file upload. You can only upload image using desktop or mobile web browser. You will need to transfer PS4 Screenshots via USB first. (See Video guide)

OR if you have 2 PSN accounts, you can transfer images from PS4 to your mobile device first by send PSN msg from one account to the other. Then you can download it through PSN Mobile App and upload these screenshot photos here.
